CNS Examination

Ni nota short case pagi tadi
Bedside buat kat IM sebab Dr tak sempat nak pergi HTAA

1. Ask for permission, introduce yourself, explain what are exactly you are going to do-failure to do so considered fail at first attemp
2. Expose adequately (cover with blanket), make sure patient comfortable and observe patient modesty
3. Start with inspection, motor,sensory and last but not least cranial nerve exam (5min is more than enough, but need more practise on it)
4. Lower limb exam done before upper limb
5. Inspection,inspect any abnormalities, look for scar etc. If patient walk, observe for gait. Then, evaluate which side is normal and always compare with the other side. So, do it right left or left right. Always start with the normal side and compare.
6. Check for muscle power, tone and bulk. Check for hip, knee and ankle joint.
7. Lastly check for reflex with hammer-knee jerk and ankle reflex. No test for hip due too deep in position.
8. To do list : Read theory on myotome and dermatome. Not to forget the most important is upper motor neuron lesion and upper motor neuron lesion.
